How would you manage a papillary thyroid carcinoma incidentally found on planning CT for T1 glottic cancer?
How do you prioritize treatment? Would you move forward with radiation to the larynx, and if so, any modification to your treatment plan?

Treat the glottis with RT 63 Gy/28 fractions and then operate the thyroid.

There's a small surgical series from Hopkins (Farrag et al., PMID 17043249) reviewing their experience with sub-TL for laryngeal Ca and they reported a 12% incidence of concurrent thyroid lesions, 6% overall of which were papillary thyroid cancer and they concluded that all patient...

Papillary thyroid carcinoma is a surgical disease with a role for adjuvant radiation (RAI) depending on risk factors, while early-stage glottic carcinomas are often addressed from the standpoint of organ preservation with definitive RT alone.
